Our CARE Program

CARE Program supports individuals living with HIV/AIDS through case management, peer support, and supportive housing services. The program helps clients stay engaged in medical care, access essential resources, and maintain stable housing to improve overall health and well-being.

About the CARE Program

At Making Opportunity Count, our CARE Program provides critical support to individuals and families affected by HIV/AIDS. We understand that living with HIV involves more than medical treatment—it requires access to social supports, resources, and a network of care to ensure stability and dignity.

Through our program, we offer case management, connections to medical providers, assistance with accessing medications, housing and nutritional supports, and help navigating benefits. We also provide education and advocacy to empower clients to live healthier, fuller lives.

Our Services

We offer a range of services designed to support individuals living with HIV/AIDS, ensuring that every person has access to the care they need in a safe, welcoming environment. 

Case Management

Dedicated case managers provide individualized support, helping clients access care, manage appointments, refer them to health insurance application assistance, and coordinate services that support long-term health.

Medical & Pharmacy Access

We connect clients to HIV-specialized medical providers and assist with navigating prescriptions through the 340B pharmacy program, ensuring affordable access to lifesaving medications.

Housing & Nutrition Support

Clients can receive referrals and assistance with housing stability, nutritional supports, and other essential needs that strengthen overall well-being.

Peer Support & Connections

Support groups, workshops, and one-on-one guidance that connect individuals with peers who share similar experiences. These services help reduce stigma, build confidence, increase awareness, and empower clients to make informed decisions about their health.

Frequently Asked Questions

No. CARE Program Services are free of charge to eligible clients.

The Massachusetts HIV Drug Assistance Program (HDAP) is a program of the Massachusetts Department of Public Health (MDPH), administered by Community Resource Initiative. HDAP helps eligible state residents living with HIV to pay for medications and health insurance.

Yes. Our case managers connect clients to housing supports, nutritional programs, and other resources based on need.

Our Impact

Last year, 95 individuals received case management and support through the CARE Program.
